Please allow me to offer you a suggestion: Drop a dime and buy a forms book for photographers. For example, the one offered by http://www.asmp.org is quite good, written by attorneys that specialize in intellectual property and last time I checked, costs non-members less than 30 bucks. There are others as well with various forms on CD that you can modify to suit your own particular needs and letterhead.
The problems in using someone elses form is that it may not suit your individual needs by not covering specific issues you need addressed or things you hadn't thought of. And if they've been modified over time by non-lawyers, you may be using a contract that contains unenforceable provisions, (like some clauses pertaining to non-refundable deposits that vary from state to state) . etc. Also, once you find a "form contract" that you feel suits your purposes, it's a good idea to run it past a local lawyer who will tell you for sure whether your contract will either hold water or stand up toward your purposes in court if need be. Lastly, remember that a form is roughly only 80% of what you need for your own specific purposes, and just about any agreement you find elsewhere will require a bit of tweaking, modification, or adjustments for one reason or another.
